Shih Tzu rescue of Minnesota accepts dogs from people in all situations. You may be moving, there may
have been a divorce or a death in a family, you may have found a stray, or you are not able to care for the
dog anymore. We also accept dogs that have health issues and behaviors we feel can be corrected if we
have an appropriate foster home available. Shih Tzu Rescue of MN is not a hospice rescue.
We encourage you to read the information provided on the “ADOPTION INFO” page of our website so
you understand the care and adoption process for the dog.
Our Owner Surrender process is as follows:
Step 1: Complete and submit our online intake questionnaire (below). Because Shih Tzu Rescue of
Minnesota puts much effort into arranging a foster home as soon as we receive the questionnaire, we ask
that you complete the form only if you definitely want Shih Tzu Rescue of Minnesota to take your Shih Tzu
and you are not currently also pursuing other options such as advertisements, finding another home with
friends or relatives, other rescues groups, etc. Once we receive your completed questionnaire we will
contact you within 24 hours to let you know if STRMN will be able to help you, or if we need further
information from you regarding the dog. Please be very thorough and honest in your responses to each
question so we can determine the best foster home for the dog.
Step 2: If, and only if, we determine we are able to take your dog into foster care, we will ask you to do
the following:
1. Make a $20.00 non-refundable surrender donation to Shih Tzu Rescue of Minnesota
2. Ask your Veterinarian for copies of the dogs Health and Medical records. STRMN does provide a
Wellness Vet Check for each dog, but we are careful not to repeat any medical procedures that are
current for the dog.
3. A Release of Ownership 2 page form will be sent to you by email. This must be completed and
either returned to us by email, or to us when the dog is picked up. We ask that you be very detailed
with the responses to the questions so we can best understand your dog’s habits and behaviors.
4. Arrange for the transport and pick up of the dog with our intake or transport coordinator
